New Patient Registration

Click here to check our Practice boundaries

If you would like to register with the practice please visit the surgery and discuss this with the reception staff. If you have your medical card then please bring it with you. Please bring along two forms of identification. One of these should have a photograph and one should be proof of address. i.e. a passport and utility bill for instance. You may also be asked about your intended length of stay, to ascertain whether you are a 'permanent' or a 'temporary' patient.

You are required to complete our new patient forms. These can be collected from reception.

We ask all patients joining the surgery to make an appointment with one of the nurses for a new patient check-up. This gives you the opportunity for a medical check-up and to discuss how we can help you with any ongoing problems.

Guide to GP Services

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.  You can download the guide below.
