Catchment area
Before filling out a registration form, check you are in our catchment area.
You are able to apply to register to be a patient of Elm Hayes Surgery if your home address is within the practice boundary.
You can also find out which GPs you can register with on the NHS website.
Registering with the surgery
If you would like to register with the Practice, please visit the surgery to collect a registration form. If you have your medical card then please bring it with you. Please bring along proof of address. You may also be asked about your intended length of stay, to ascertain whether you are a ‘permanent’ or a ‘temporary’ patient.

We are able to offer registration for patients who live outside of our normal practice area under the out of area registration scheme in exceptional circumstances. If you require an out of area registration or are an existing patient who moves outside of our area you will need to forward a request by email to reception.elmhayes@nhs.net for consideration.

Temporary residents
If you are not registered with a doctor but need to see one, you can receive emergency treatment for 14 days. After 14 days you will need to register as a temporary or permanent patient.
To register as a temporary resident you will need to complete a temporary resident form, which is available from reception.
You can register as a temporary resident with a GP in England if you are in the area for longer than 24 hours but less than three months.
