New Patients

You are able to apply to register to be a patient of Elm Hayes Surgery if your home address is within the Practice Boundary.

If you would like to register with the practice please visit the surgery and discuss this with the reception staff. If you have your medical card then please bring it with you. Please bring along two forms of identification. One of these should have a photograph and one should be proof of address. i.e. a passport and utility bill for instance. You may also be asked about your intended length of stay, to ascertain whether you are a ‘permanent’ or a ‘temporary’ patient.

You are required to complete our new patient forms. These can be collected from reception.

We ask all patients joining the surgery to make an appointment with one of the nurses for a new patient check-up. This gives you the opportunity for a medical check-up and to discuss how we can help you with any ongoing problems.

We are able to offer registration for patients who live outside of our normal practice area under the Out of Area Registration Scheme in exceptional circumstances.  If you require an Out of Area Registration or are an existing patient who moves outside of our area you will need to forward a request by email to reception.elmhayes@nhs.net for consideration.
